Focused ultrasound ANT ablation for refractory epilepsy: pilot safety and feasibility study

Summary

A pilot open-label clinical trial of MRI-guided focused ultrasound ablation targeting the anterior nucleus of the thalamus in two treatment-refractory focal-onset epilepsy patients using the Insightec Exablate Neuro system. Safety was confirmed with no new neurological deficits or neuropsychological performance deterioration at 12 months. Both patients showed meaningful reductions in seizure frequency, establishing proof-of-concept for non-invasive ANT ablation as an alternative to implanted ANT-DBS.

Why it matters

ANT-DBS is FDA-approved for drug-resistant epilepsy requiring implanted hardware. Demonstrating that non-invasive focused ultrasound ANT ablation can achieve similar seizure reduction without surgery opens the possibility of treating epilepsy patients who are poor surgical candidates or who prefer non-invasive approaches โ€” a substantially larger patient population than current DBS surgery candidates.
