Closed-loop spinal cord stimulation restores walking in chronic spinal cord injury

Summary
Three patients with chronic incomplete spinal cord injury regained overground walking ability using epidural spinal cord stimulation with real-time EMG feedback to adapt stimulation parameters to gait phase. All three patients could walk outdoors with a walker after 5 months of stimulation-assisted training; two maintained partial recovery after stimulator removal.
Why it matters
Recovery of voluntary locomotion in chronic SCI patients had not previously been demonstrated. This study shows that activity-dependent neuroplasticity enabled by closed-loop SCS can produce lasting recovery, not just device-dependent compensation.
